Every holiday season, people give thanks for their health and well being and every New Year, we pledge to take better care of our health and well being. It’s a nice holiday sentiment, but by about January 7, we’ve likely forgotten about it.
This is human nature, of course. During a time of thanks and gift giving, we are reminded of the things we often take for granted. Then, when those times are past and we’re back to the daily grind, those thoughts get moved to the background as our daily lives take their place. For many Americans, many of whom are dealing with silent health issues that are crying for attention in our busy lives, the day-to-day lives we lead have little room for health issues.

Personal Health Records and Open Notes
A patient’s personal health record (PHR) usually contains things like test results, doctor’s recommendations, prescriptions, office visits and their purpose, diagnosis, and so forth. Sometimes, but not usually, they can contain the doctor’s notations and observations. Traditionally, these Physician’s Notes (which can be made by any health care provider, in practice) are not often open to the patient and are usually not distributed outside of the individual medical practice or hospital. There is some debate about the idea of Open Notes & Physician’s Notes that are openly available to the patient and/or a PHR.
